She's not good at what she does, but your intuition is correct that it's more difficult to do surgery, whatever the specific procedure, on fatties.
First, there's the matter of correct intraoperative positioning. Obviously, the surgeon doesn't do the heavy lifting, but in order to maintain an airway, a fat patient sometimes has to be positioned in a way that is suboptimal for ergonomic access to the surgical site. Musculoskeletal disorders and chronic pain are significant issues among surgeons, especially those who perform mostly laparoscopic procedures. This article is a preprint, but it's quite fascinating if you're into this kind of thing. The authors used Xbox Kinect and a wearable device called an inertial measurement unit to capture joint movement and postural load on the upper body during a simulated laparoscopic procedure. They found that surgeon posture degraded with increasing patient body mass index. Procedures involving the thorax were particularly likely to increase musculoskeletal workload, even for "expert" surgeons (those with >=10 years' experience and >100 laparoscopic procedures performed annually).
Laparoscopy is considered the safest method of surgical access in the obese patient. In a laparoscopic procedure, access to the operative site is accomplished via instruments called trocars, which are used to puncture the skin to allow the introduction of a long, hollow tube called a cannula. The cannula is how surgical instruments are introduced without making a large incision. Obviously, placement of these is important for both patient safety and surgeon ergonomics. In obese patients, trocars may have to be placed in nonstandard locations, which is dangerous for the patient and requires the surgeon to adopt an unnatural posture, risking injury. The surgical table also has to be wider for a bariatric patient, requiring the surgeon to reach further to see and access the surgical site.
In addition, procedure duration is often greater for obese patients than for those with normal body mass index, meaning the surgeon has to maintain whatever uncomfortable posture he's forced to adopt for more time. The article I mentioned above discusses the impact of surgeon experience level on the duration of a procedure and in turn, the amount of time the joints may be subjected to undue physical load as a result of poor posture. The authors found that novice and intermediately experienced surgeons spent longer completing the simulated surgery, meaning their upper bodies where subject to detrimental biomechanical force for longer. All surgeons were slower as "patient" body mass increased. I think with Dr. Gallagher's level of experience, she's probably not as readily able to compensate for patient body size, and her posture suffers. Part of the problem, whether she realizes it or not, may be that by the time she gets to the closure portion of the surgery, she's uncomfortable and just wants to be done with it. Of course, that's absolutely not an excuse, and she sucks in other ways, but it makes me wonder.
Some of the authors of the preprint article have another paper in the Journal of Minimally Invasive Gynecology if anyone would like more to chew on. I've only skimmed that one but it seems like the findings were similar. This is not even beginning to get into how obesity affects wound healing, but I think that's been covered in other posts in this thread and the general SRS/GRS horrors thread.

If blue-outfit were my daughter I would sterilise/put her down myself. And I'm not (just) trying to be edgy here - there is no way that girl will survive and have a worthwhile life after her parents die/become too old to take care of her. Maybe, maybe if she has siblings who are willing to go above and beyond, but they usually have their own lives and families and can't take on an adult disaster like that.
I can't imagine the horror of having a troon disaster child. It's hard enough when your child is physically disabled in some way, but at least in most cases there is some hope and life and joy there. The type of mental illness that causes a girl to do this to herself before she's even 25 is a pit of despair.

Weirdly enough, there was a sitcom called The Mick* that had a solid grasp on the whole "trans kids" phenomenon. One of the main characters (and one of the best parts of the show) was a 10yo boy named Ben who really liked wearing girls' clothing. There was an entire episode - S01E11 The New Girl - that was basically a commentary on that. Ben's just a boy who wears girls' clothing because it's what he likes. Nothing more to it. It also showed Ben not actually knowing what being transgender even means.
Honestly, I just really like that episode. And that show. "The New Girl" is a great watch if you're looking for some goddamn sanity.

I think they're cheesing so hard because they're convinced they've just saved their child's life.
Occasionally, you'll see the parents of a childhood cancer patient ask to take photos of or with the bag of stem cells the child is about to receive. Stem cell transplant remains the only shot at cure many kids have, and even though the preparation is absolutely unimaginable to watch, the infusion itself is kind of anticlimactic. Even though they've just been through hell and their child, worse than that, there's still this instinct to commemorate the moment. They smile because they're immensely relieved that the child has survived incredibly intense chemotherapy in preparation for this moment, and because the little bag of goo represents all of their dreams about the future.
I really think that a lot of these parents have been browbeaten into believing that a "teet yeet" or stinkditch surgery is like a stem cell transplant, something brutal and painful their child has to get through in order to fix a life-threatening medical condition. The happy "after" photos are because they really believe the worst is behind them and their baby is cured. Like the parents of the kid receiving stem cells, they're looking toward a hopeful future.
Unfortunately, some kids relapse post-transplant, and when that happens, the prognosis is not very good. This section of the Farms abounds with young people who were sold on the lie that their problems could be cured with a surgical procedure or two, and that even though the recovery would be extremely painful and difficult, it would all pay off once they were free to live as they deserved. Just like a child who relapses shortly after treatment, many of these troons are forced to reckon with the surgery not being the panacea they were told it would be, and once they've chosen to go through with it, how many options are truly left?
Pretty grim, all told.

This really could be a horror/dark comedy sequel parody to one of those stereotypical teen movies. That kind of research cannot get funding, backing, publishing, etc. Even attempting to approach it is like wearing a massive neon sign reading PLEASE RUIN MY ACADEMIC CAREER ASAP. Just like any serious in-depth research about detransitioners, mental health after SRS, frequent comorbities with transgenderism (autism, DID, paraphilias, PTSD from sexual trauma, etc), and so on.
