He has no muscle mass to speak of, probably none of note since puberty ended. The constant alcohol binging is giving him little more than a constant stream of sugar for energy, I guarantee you he's not getting anywhere near a decent level of nutrients outside of salt. The fat stores from when he used to eat actual food have some nutrients stored in them, so his body is burning the fat to release those, all while bogged down by the alcohol.
