[Feminizing genitoplasty for prepubertal children and teenagers female]​

Objective: The aim of this study was to report our experience after 10 years of practice of feminizing genitoplasty in prepubertal and adolescent patients with disorders of sex development (DSD) assigned females as females in a developing country.

Methodology: This was a cross-sectional, descriptive and retrospective study over a period of 9 years. All pre-pubertal (8-12 years) and adolescent patients female sex assigned with DSD who had willfully consented to the surgery with their guardians and underwent feminizing genital surgery were enrolled in the study. Data collection included: age at presentation, precise diagnosis, surgical procedures, complications, cosmetic result and duration of follow-up. Each patient had a precise diagnosis and the surgery was planned after discussion with the multidisciplinary team. Cosmetic results were assessed based on: appearance of the clitoris and separation of the vaginal and urethral openings.

Results: Nine patients raised as females with a median age of 8 years (IR: 10.75) were recorded. Surgery was performed at a median age of 11 years (IR: 9.5). In this series, 6 had a 46, XY karyotype with varying diagnoses: partial androgen insensitivity syndrome (n=2); 5-alphareductase insufficiency (n=2); 17-ketoreductase insufficiency (n=2); gonadal dysgenesis with a mutation in the NR5A1 gene (n=2), 2 had ovostesticular DSD, (karyotypes 46, XX), and 1 had mixed gonadal dysgenesis (karyotype 45, X/46, XY). Partial or total gonad(s) removal in accordance with assigned gender was the most common associated procedure. It was bilateral in 7 cases and unilateral in 2 cases. Follow-up ranged from 3 months to 4.5 years (median: 26 months, IR:18.25). One patient had acute urinary retention in the early follow-up. No other complication such as incision bleeding was recorded. The cosmetic appearance of the external genitalia was satisfactory in all patients.

Conclusion: Feminizing genital surgery in Cameroon remains a major challenge and should seldom be realized without a precise diagnosis. Late age at presentation is peculiar to our setting; however, it gives room for the patients' participation and input to decisions that will have a life-long personal impact on their lives in terms of psychosocial development and fertility.

TDLR: 6 genetic males with 'unacceptable' male genitalia and 3 genuine hermaphrodites (including a possible retard) raised as girls in Cameroon get genitoplasties at 11 years of age.

• 2 had partial androgen insensitivity syndrome
  • (genetically male with a lower testosterone count due to some insensitivity to androgen)
• 2 had 5-alphareductase insufficiency
  • (genetically male, don't produce enough DHT, similar to above)
• 2 had 17-ketoreductase insufficiency
  • (genetically male, usually have phallic enlargement, male secondary sexual characteristics as teenagers)

  • (genetically female, has male appearing genitalia, considered true hermaphrodites)

  • (mosaic turner syndrome/male mix, extremely rare but has lots of symptoms including retardation see here )

The last three patients here have syndromes that tend to be more male presenting (male habitus) despite being the more genetically female. Absolutely none of the 9 patients here should have been surgically altered as minors.

My 600-lb Life: DAE feel like you are watching what could have been your story?

I've always liked watching My 600-lb Life. When I still lived with my parents, we would watch it together sometimes. They would always gawk and comment on the amount of food, the candidates attitude etc. I understand that this is probably the main reaction from the general audience, but it always made me feel sort of bad.

I grew up fat with fat parents. I have since become very physically fit. My parents lost some weight, became a little more active, but remained overweight and are okay with that. When I watch the show, I mostly feel sad and empathetic towards the contestants. Mostly because I feel like I am really not that different from them. I feel like if my circumstances would have been different, I could have gone down that path too. My parents will say: ''God, how do people let themselves go like that?'' but it's just... not that unthinkable to me, I suppose. I know how it feels to be obese, I know what it's like to have an entirely obese family. It's just not that hard to imagine that a similar situation can escalate to the things we see on the show.

Sometimes I half-jokingly say that I feel like an obese person trapped in a fit persons body. See, my ED absolutely changed my body a lot, and these days I am thankfully doing pretty well, even somewhat in recovery. But I still often feel like on the inside, I'm still that obese person.

I guess in a way it feels like I am looking at what could have been my life. I was never as obese as the people on the show, I don't think my BMI ever went above 38, but still. Under different circumstances, I could have seen myself ending up in a similar place.

I wonder if anyone can relate to this and has similar feelings when watching this show. I'm sure I'm not the only one! I would love to hear what kind of thoughts and feelings watching this show brings up for others.

Different people like different things. And those things can even vary based on the individual partner or sexual activity. There is no universal answer to this.

I am not a doctor, this is just my personal understanding of my anatomy based on what my doctor told me and what I read online.

I didn't have my V closed, I had it entirely removed. The pelvic floor muscles remained in place, of course. And on the inside there's also the internal parts of the clitoris, and the skenes gland which may play part in my sensation.

When my bf penetrates me, it stimulates the muscles that used to be around the V, and what feels like the internal parts of my clit and what I think must be my skenes gland. I don't really feel erotic pleasure in my anus itself, but the movements stimulate the erotic tissue around it. I think when I was pre-op, it just wasn't really in the right place for that to happen, but now it is.

It may also have something to do with my balls. When I pull my balls forward, or when they slap back and forth, it also stimulates the area. Probably because my clit is buried somewhere in there. But I definitely think it may be something to do with the skenes gland as well, from my understanding it is similar to the prostate and some ftm people get lucky as in that it grows from hormone use.

Every interview that I have ever seen with people who had any kind of intersex condition has said that the most traumatic thing was not having the condition, but being lied to about it, and they said that any medically unnecessary surgery should not be done until and unless the person wants it.

I went to the urologist … He was dilating the stricture at the tip and was putting in a catheter to keep it open. The catheter wouldn’t go in all the way because -lucky me- i have another stricture. Except the urologist couldn’t fix this one. There was a buildup of scar tissue near the base where i had a fistula. He didn’t want to try and dilate it because he thought there was a great chance he’d make it worse by recreating the fistula. He said that he really didn’t know what to do about it and it wasn’t something he could handle. He said he would call Dr Crane and explain the problem.

I swear her arms are weirdly short. If you check out the first tiktok she can't seem put her hand properly in her pant pocket because her arms can't reach. She has to pull her pants upward to do so.
