Either fix it straight away when the mistake happens, or you're kind of stuck. At least in Abostralia, going through the name change bullshit is a huge headache and adds at least an hour to any significant paper work as you need documents to elucidate the name change, and said documents must be signed by a Justice of Peace or pharmacist, who would demand the original documents.
Now imagine doing this for every legal document, cannot blame them for wanting to save an hour each time. It just shows how inefficient the government can be.
