In Pat's case, the shoulder seam goes pretty far down his arm, so no, sizing up isn't the fix, he's actually wearing a shirt that is slightly technically too large. That seems very counterintuitive, but if you look at both pictures, there is actually excess fabric in certain places, despite it being very tight in other spots.

No, what our customer needs is a FBA, a full bust adjustment. Ignore him bleating at you that it's his pecs that are big, no if that was the case, the pull along the shoulder seam would be more even. His tits have a very prominent apex that you don't get with muscular men. That's why the pull lines radiate around his neck like that.
