We can't know for sure. The fact that there is no research on the side effects or outcomes of totally preventing puberty is one of the reasons people oppose this bullshit.

However, my educated guess is that if he discontinued the estrogen supplementation and started an appropriate dose of testosterone, he would improve physically and mentally. 2/3 of our genes are expressed based on our chromosomes and genetically we "expect" the congruent hormones to be present. Testosterone would likely cause him to lose a significant amount of weight and would likely also improve his mental health.

He wouldn't have testicles or a dick, but he could still have a chance to be a healthy man. Plenty of men have lost their junk in wars or farming equipment accidents and they're still healthy, normal men. The real question mark is whether he would be able to mature mentally at all if he went on testosterone. My guess is no, he will always have the mind of a child no matter which hormones he takes. Everything we know about puberty suggests that it's really a one-time window. But having the hormones his body expects and wants could not hurt him, even though it wouldn't fully "fix" him, and may provide a significant benefit to him both physically and mentally.

It's a shame he will never have a chance, since with the mind of a child he's always going to be under his mother's thumb.

This idea that gender-transitioning mentally ill children and young people will somehow help them and will prevent suicide is just provably wrong. The affirmation and celebration that these young people got was overwhelming, yet their parents still insist that they died due to "transphobia."
