I can't see any obvious signs of infection (unless she's plastering on more make-up to cover the redness) and that's boggling my mind.

I am not a burger so idk how it works over there but in my part of Kiwiland they'd record that that way because it's a self-reported history of such, rather than a medically diagnosed one. So could just be that she tells them she starves herself and they raise an eyebrow and say "Sure ok bb, we'll put that in your notes".
