I've never heard of fistula like this before. It's mostly in conjunction with a vaginoplasty where the vaginal canal is punctured in the colon allowing a variety of serious complications, including sepsis.
How does this compare in level of potential harm?

If do indeed have urine escaping from the location in the photograph, then you should definitely be reaching out to your surgeon as that could lead to other complications. I would also avoid putting anything like medihoney there until you have confirmed with them that it is OK.
I am not a doctor, and the photo is a bit blurry, but compared to some of the other dehiscence photos people have shared - and what it ultimately looked like once fully healed - that doesn't look too bad. Your concern over a potential fistula however, based on your comment, does seem justified.

It took me 2 years after top surgery to realise I had no connection with my lower half. I don't acknowledge that part of my anatomy.

It took another 1.5 years to realise I will never be comfortable with intimacy in my current body. I would be trying to find a life partner if I had a dick. I want to grow old with a dick.

The surgeon called me directly to say how shocked they were, because they found out on the operating table that my bits were smaller than a cutie orange, and they couldn't get anything through the hole. There was never a reason for an exam, so nobody knew I had a congenital defect down there. All of my bleeding was from cutting through that complication, not the hysto.

My theory on this is some kind of unusually tough or imperforate hymen. That would explain why she was "never comfortable with intimacy" (because she couldn't actually fit anything in her vagina without blood and pain), would count as a congenital defect and would cause a lot of of bleeding when cut. Albeit I don't believe the surgeons were stunned by that, it's not exactly a one in a million complication.
