It's hard to be sure from such a shitty photo, but it honestly looks like he has a congenital anomaly called triphalangeal thumb, which is just an extra bone in the thumb causing it to appear more like a finger. Not a malformation one sees very often, but it can happen in isolation or as part of a broader syndrome.

I want to see more photos of his hands. Fascinating.

You found it! Went through a video found the building by the road and compared it to the google map building. Same windows, roof slope, color, door and roof light. Great find!
