Unless in this hypothetical you came down with an acute overnight case of sudden onset progeria, the mismatch between reality and perception is in your mind, not in your body. Either you're seeing yourself old and you're not (like an anorexic sees themselves fat) or you're old and you never updated your internal self-image (remember all these women who had mental breakdowns when a photo filter made them look like teens or on their 20's?) or you forgot your age (maybe you've got alzheimers or dementia).
So what you do is you confirm your perception with other people.
And people will tell you "no, you're not 90, you look your 20s, you need to see a therapist, or maybe an oftalmologist or neurologist". Or they'll tell you "grandpa, we've been through this, you're 90, I'm your grandson, you just get confused sometimes, let's get you your pills".
But imagine if society had decided we had to tell that 20 year old "yeah you're 90, grandpa!! Let's give you a hip replacement and blood pressure medication (which you don't really need) or that 90 year old "yeah you're 20, we need to give you a facelift IMMEDIATELY! (You'll probably die in surgery because YOU'RE FUCKING 90)".
That's what we're doing when we indulge and reinforce the misconceptions of these confused people. Sure honey you're a brave and valid woman; of course dude you're such a manly man.
And that's just the sincere ones. Not the ones faking it for access to spaces or for social capital or to escape trauma or for the cums.
