Those with narcissistic personalities are not really capable of accurately predicting what their "enemies" will do in response. They spend no time putting themselves in someone else's place. They think they can create the perfect plan that will get everyone on their side just by controlling the inputs, and if they just shut up the people who are talking about them, then they'll come out on top. So every time someone responds in a way they didn't predict, even if it's just laughing in their face, it is a brutal surprise for them and creates immediate grief-rage.
Every time another person exhibits agency, he melts down all over again and doubles down on his efforts to control them. It doesn't die down because everything is a trigger, and there isn't an obvious point where he would suffer consequences for it and try another method of being okay with himself.
