To extend a massive benefit of the doubt- I wouldn't completely discount Thyroid issues. While Hypo- and Hyper- are well-known issues, there are things like Graves Disease that can complicate diagnosis. It trends towards hyper but medication is so ineffectual you frequently wind up jumping straight from doses that do nothing to completely shutting things down and triggering hypo. Generally the last treatment for this is RF Ablation of the thyroid and then artificial hormones to maintain normal levels.
