Unironically the Three Stooges thing could be true. If it was only because of some other fucked up shit that they found the widowmaker blockage (left main artery or left anterior descending artery), then the other thing may have accidentally saved his life (for the moment). If you have a heart attack from that blockage you pretty much just always die.

You’d be surprised how many afflictions a human body can suffer simultaneously and not die. I mean look at the Slaton sisters and others who weigh almost 1,000 pounds AND smoke AND are 100% sedentary, AND get constant UTIs, staph infections, Covid, you fucking name it.
Sometimes being kept alive to suffer all these is a more brutal punishment than death would be. I’ve known a few people who were in similar situations and fortunately they all found a way to accept reality without becoming embittered. They took the good with the bad and their gratitude elevated bad to survivable and good to great. (Those who claim Jim’s faking are basing that on the fact that they all know they’d be throwing tantrums and pity parties if they were in Jim’s shoes. But he’s not playing victim and is actually poking fun at the situation so he must be faking cancer.)
Jim might be a cynic but he doesn’t seem to be sitting around pounding his chest and asking “Why me?!” If that absence of pointless agita counts for anything — and supposedly it does — it will help keep him alive.

I looked up vertebral artery dissection and apparently it's a kind of stroke. So at this point, he's had cancer, a stroke, autoimmune issues, arthritis (I guess caused by the autoimmune stuff), osteoporosis, kidney stones, and now a heart attack.
And he's only now quitting smoking?
