I get the impression the later is the bigger fear since that's their safety net, their "friends", the people they try to impress, and the network they are terrified of losing after ditchingall their other friends for supportive ones. Surgeons (usually) eventually stop being in your life, the cool kids you're terrified of disappointed are in your brain forever.
