So how many more people does Matthew have to abuse and then threaten with his own death after they leave, before women start avoiding him? It amazes me how he continually manages to not be entirely socially outcast with how unstable and disgusting of a person he is.

Contrary to Instagram-filtered "reality" and the endless pics of people that troon out before male puberty, it's actually quite hard to undo it after it changed your body.
Naia could well be using HRT, and that's the extent of HRT effects on him. He probably got some horrific male tits and fat redistribution, and chemically castrated himself - while also remaining with a moustache, beard and the angular face of a male.
