Along with all the other ways it fries your brain, heavy alcohol consumption can (sort of ironically) make you more anxious and paranoid long-term. It's one of the reasons alcoholism is so insidious, you drink to get rid of anxieties and end up making them worse in the end. Some of Pat's behaviour is definitely unhinged enough to be him abusing uppers, but it could also easily be the booze plus his withered soul finally departing his body once he paid Quasi, leaving him a broken shell of aOh my.... Well, if his brain wasn't broken before, it certainly is now. I have seen thinking like this before and it was meth-psychosis. I knew a guy who also thought random people were after him, plotting to kill him and such, and it was because he had been doing meth for 2 weeks strait and cooked his brain. Does Nikki have a script for Adderal? Not even joking here, this kind of thinking can be the result of doing too much speed. It has already been said here, if he hears and Ice-cream truck, he just might start shooting.
