nose mouth, and ears are waaayy too huge for a woman, jaw is protruding and their is way too much spacing from the tip of the lips to the vertical eyeline, stubble, hairline obviously unnatural, mandible angle is way too pronounced, everything looks like what happens when I get frustrated at photoshop's liquidify tool.
Here's a comparison to Anne-Hathaway, someone with a typically-proportioned face, just to give you an idea of how stupid his proportions look.

Not to PL too much, but a friend of mine married a girl of Portuguese ancestry from the New Bedford area (Fair Haven) who was shorter, fatter and hairier than elephant dick started out. He is a 6'3" American of old Yankee stock. They are divorced now (he is on his third wife) but they had identical twin daughters who had the misfortune of being in the 95th percentile for weight and 99th percentile for height when they entered kindergarten. These girls are going to be behemoths, but haven't shown any genderspecial tendencies as of yet. Probably won't, most pooners are tiny.

Experienced a bit of wound dehiscence as my stitches all kinda starting coming apart after the last update (week1) but they feel and look to be filling in. Nicely
I've been very diligent with my wound care and I think it's showing.
Still very swollen, especially on the left side.
I apparently should have been on Oxy for this but I hadn't and my icing wasn't very diligent cause it caused more pain, but now that I have some I've been able to ice.

New Phalloplasty Method: PIPE (Phi Innervated Periumbilical Eversion) Phalloplasty

A few hours ago, I received an email from Mass General Brigham detailing a new phalloplasty method Dr. O'brien-Coon is offering called Phi Innervated Periumbilical Eversion Phalloplasty (or PIPE phalloplasty for short, which imo has to be the funniest name for a penile construction surgery).

It's a form of abdominal phalloplasty that uses the epigastric artery and vein to create a penis using the skin above the belly button as a donor site. From my understanding, this differs from other kinds of abdominal phalloplasty because of a few reasons:

1. The penis is created using a free flap rather than a pedicled flap.
2. An artery is used to supply blood to the penis rather than relying on the attached skin of a pedicled flap.
3. The penile scar will run on the underside of the penis. Often times abdominal phalloplasties place the penile scar on top or on the side of the penis.
4. It forgoes the need for a RFF flap for urethral lengthening or nerve hookup (something other abdominal phalloplasties may need).
5. No placement limitations as it is a free flap.

The phalloplasty staging is as follows:
Delayed flap preparation than 3-4 weeks after stage 1 phallus creation 3-6 months after that stage 2 perineal masculation with or without UL than 3-6 months after tubularization* and glansplasty than 3-6 months after stage 3 implant placement.

*I don't know what this means. If anyone does please comment. Big thanks.

According to the pdf, the donor site can closed as a horizontal line scar. However,someone in this subreddit had this procedure who's scar looked more like an upside down L. I assume the added vertical scar was for harvesting the artery, but don't quote me. I'm not name dropping the guy but if he speaks up he speaks up.

Anyway while the donor site can be closed as a line scar, it might not be achievable for thinner patients. Tissue expanders would need to be used later on to achieve this. It does not say what the scar could look like without a tissue expander for these patients.

OH and this technique was advertised to me as a method that forgoes electrolysis. Although, I think this was with the assumption that the patient isn't like super jumanji hairy on the upper abdominal region, but unfortunately it does not go into detail regarding hair removal or electrolysis.

Sensation wise, this technique is inferior to RFF but allegedly it's sensation capabilities will be similar to ALT, however, there is no long term data to say for sure.

All the information I've listed is from the email and file provided, and I'm unable to ask any more technical questions until my consultation. My consult is in May so I guess I'm out in the rain until than

Alright so lets look at this new method.

Phi Innervated Periumbilical Eversion Phalloplasty (PIPE phalloplasty) by Dr. O'brien-Coon​

In u/starsanddrowning's words, it uses the epigastric artery and vein to create a penis using the skin above the belly button as a donor site.

5 planned surgeries: (time between the next surgery in brackets)

1. prep surgery (3-4 weeks)
2. phallus surgery (3-6 months)
3. perineal masculation and UL (3-6 months)
4. tubularization and glansplasty (3-6 months)
5. implant placement

So ideal time frame is ~10 months and the non-ideal is ~1 year and 7 months. Unaccounting for any extra surgeries that are bound to happen and the last surgeries recovery time.

Features:

• Free flap. This means both the tissue and the artery and veins are completely cut away (as opposed to ElephantDicks need to have repeated surgeries to slowly inch it down.)
• Scar will be facing downward as they can choose the orientation with free flaps procedures. (Cetrulo's abdominal had it on the side and the standard type has it on top.)
• No other grafts needed from arm.
• Use of artery so less risk of necrosis. (I think Cetrulo just used a vein)
• Gets split apart for UL and needs to be restitched up in the next surgery (I think, going by the "tubularization" stage)
• Can be closed with a line scar unless you're very thin (as opposed to ElephantDick's massive wound)
• Tissue expanders for the thin ones.
• Sensation is worse than rff
• No electroysis needed

Artery and vein used are shown in this picture.

Periumbilical is just the area used and eversion means to turn inside out. innervated means to supply nerves to.
I couldn't find exactly what the Phi might mean here (something to do with nerves) but maybe he was just being fancy with using phi instead of the word phallus? Either that or this is a coincidence:

Spoiler: Is it true that you have to regularly douche your Neovagina to get rid of dead skin cells and such, even if you're fully healed?

I've read comments from multiple people that say if you get PI you have to clean the Neovagina like once a week, or else dead skin cells from the tissue will cause problems.
I knew you have to douche regularly for the first few months or so, but they said you have to basically do it forever, is that true?
The fear of this actually being the case and thus never getting to build up a natural vaginal flora down there is causing me a lot of anxiety

Let the lining of your vagina adjust and it’ll become more of a mucosa over time, and other than using a lubricant for intercourse, there’s no real reason to douche unless your gynecologist tells you to after a vaginal infection.

DeannaWilliams222: I use good clean love restore lube. Never have to douche to "flush out lube or skin cells".I also use good clean love bacterial suppositories called biophresh which helped seed lactobacillus bacteria in my vagina.​

Douching is actively harmful to maintaining a good bacterial colony in your vagina. This is true for cis women and for trans women. It disrupts the pH balance and physical flushes bacteria out. Douching is only helpful when you actively have high pH discharge happening inside the vagina, such as during healing after surgery, but even then I had far greater success using suppositories every few days rather than douching...​

I do not get "built up skin cells" in my vagina. Dilating, sex, and masturbation all help with moving dead cells out of the vagina.​

Oh... I also use balance feminine wash (also a good clean love product) when I shower. I clean my vulva and use a finger in the entrance.​

Shana_tan: My GP gives me Aquagel for dialation and general lubrication down there (PI), I would douche after using it internally; is this a bad thing ultimately? I did have to use some prolactin suppositories to try get a good biome there a few months ago as it was getting really smelly and it's been much better since.​

The main reason cis women avoid douching often is because the solutions they market are full of fragrances and irritants. Lysol was marketed as a douching solution when it first came out. It’s not necessary for most people but it’s a different situation.​

Putting some diluted vinegar or saline etc in your neovagina will do nothing to hurt you.​

She does the PPT method and, of course, everything that's in her docs is likely based on what they feel is the safest course of action, so others experience is equally important to this conversation.

But according to the aftercare summary alone from her office, yes, you should douche 1-2 times a week pretty much for life.

Again, that's her office's recommendation and specific to her procedures though.

My surgeon didn't tell me that i would have to douche for life. In fact they made it seem like i would only have to douche for a few months after surgery.

Unfortunately i have now been told by the same surgical team that i need to douche it once a week at a minimum or it will continue to smell awful. They even said to use betadine if i can't stand the stench that comes from it.

I have the crap simple PI so there is no mucosal tissue up there. The dead skin has nowhere to go, it just sits up there going rank until i wash it out.

If you managed to grow a biome that doesn't smell disgusting then congratulations. I have tried, mine is obviously just crap.

18 months post op and it just keeps getting worse. Wish i had died before i even got the surgery instead of suffering every day since i had it done. I can't even just ignore it as the smell is awful and I'm sure other people can smell it through my clothes.
