While you can survive in good health even if you are homeless (there are many programs designed to help the homeless not die on the street), you need to be very, very hygienic, spending more time on hygiene than if you were not homeless, because you are constantly in a dirty environment, without easy access to medical care or sometimes even clean water.

Obviously, Daniel is not hygienic in any way, so he has lost most of his teeth, gotten scabies, developed multiple foot infection, developed multiple UTIs, among many other things. Despite people worrying about scabies and a foot infection, I think Daniel's biggest health risk is actually dental. Lacking a full set of teeth/dentures has disastrous effects towards the rest of your body, and once you lose your teeth, they are gone forever. Furthermore, his dental hygiene is the most likely contributor to his extremely weak form, as he would likely be malnourished if he has not received professional dental care.
