Consistent with lying about his age, the comic claims that this happened after 2000 (with the dot-com bubble and all) but apparently he told his ex 1998. That alone raises doubts about its authenticity.

Cave Deli is the pseudonym he used when drawing "adult" comics for the Stile Project around 2003. He was paid to draw a webcomic series hosted on there called Stile Sux and also drew various ads for the site. It was pretty mediocre work (even for Shmorky) and mostly was his self-insert complaining about working for real-life employer Stile. Seems like a theme for him, really.
His self-insert from the comic is on the left here, I couldn't find any examples of the actual comic anywhere.

Yeah, I'd have to agree that's his actual voice. There's constant comments nowadays about how his Tranny Urkel voice was his real voice, but in this video the closest thing you get to hearing the current "cutesy" tone of voice he obsessively uses nowadays is when he's impersonating the annoying kid in the video they're commentating over. His voice is nasally in the other older videos he's in, not as gratingly high pitched and gross as it is now.
