I saw a human interest type article that mentioned it a loong time ago. I can't recall where or when. Sorry.

But I did do a quickie search on Duck Duck using "transgender dementia patients forget their transition" and got some decent looking results. Pubmed. Sagepub. And some Alz sites. I didn't look closely, so don't blame me if they all parrot each other. Or if you need to register or pay for access.

This one looked promising: https://journals.sagepub.com/doi/full/10.1177/1363460719876835 If anything, there's a bibliography of sorts on the bottom with researcher names and whatnot to chase down.

It is fascinating. And it rings true. Alz/dementia sufferers often lose decades. Blanking out on your transition could happen.

It could be lens distortion, given the legs look a bit out of proportion as well.

Also as a charitable note, if the clownshoes are real and intentional there might be a non-fetish reason for it: if one has feet that are wide to the point of making shoe shopping a pain, sometimes you can go up a few sizes on an otherwise narrow sneaker design and end up with way too much toe but otherwise very well fitting shoes since the beginning of the widening in the ball area in the larger size shoe can perfectly fit the widest part of the otherwise rather short foot, and if the rest of the shoe doesn't slip plus you walk in a way that you don't constantly trip on the toe, you're golden.

I won't 'cure' a pre-existing mental illness. Both dementia sufferers and (severely) mentally ill people have de facto regressed to a young child's mental or cognitive level. There are certain differences between mental illness and dementia and how it manifests (MI, especially the severe types that result in psychosis, mainly impact sensory perception and how the brain sorts and categorizes information, whilst dementia in its early phases mainly impacts working memory and things like spatial reasoning) but if one already has MI dementia exacerbates the symptoms rather than cure the patient of them.

Frontal lobe dementia in its early stages can mimic cluster B personality disorders with frightening accuracy. If a previously normal 50-something suddenly starts to have histrionic chimp-outs like a borderline mess, there is always a reason to investigate frontal lobe dementia.
