Its due to the nature of operating in that area combined with sutures. All surgery involves trauma that causes inflammation & swelling however its more pronounced in some areas and some people have varying degrees of said inflammation and swelling. It dies down as the healing process runs its course but you're definitely going to get unsightly results if you take pics right after surgery , even months afterwords like most examples on here.
