Hi! I (24 trans f) had a vaginoplasty in late June with Dr. Brassard. I was in constant pain for about 5 months, and active bleeding happened for that length of time too. I have limited pain now (can just take some Tylenol and Advil finally, rather than needing opioids). The staff at GrS have been less than helpful at all times and ignored a very active infection early on. They also never scheduled a follow up to date while ignoring calls/emails. So, seriously: I’ve been dumped after my surgery. I haven’t seen my surgeon since before the procedure! I’ve complained to the appropriate health ministry about this and other problems, and it’s under investigation.

That’s not the reason for this post, however. It’s over the results of my surgery. I received no labia minora. The nursing staff emailed me back a photo saying it was “small” but are you serious??? It’s nonexistent. Please check and tell me I’m not making this up!

1 year post-op phallus-preserving vaginoplasty
My 8 month
It has been one hell of a journey, that's for sure! Some major milestones since the last post:

I've successfully bottomed for sex! General activity and mobility are pretty much back to normal. No more constant, persistent pain.

I still have some granulation, unfortunately, and due to a lot of tightness, I'm still dilating twice a day until that scar tissue softens up more. I've been able to maintain about 4.5" of depth which is plenty for sex and should also improve when scar tissue softens more.

Here's some answers to some frequently asked questions that I know a bunch of you will still ask anyway. My surgeon was Dr Gurjala at Align Surgical in San Francisco. My vaginal canal was made using a mesh graft of my scrotum. Yes, I can still get hard, still top and cum from my penis and my vagina. I use testosterone gel to maintain penile function.

I can’t even begin to tell you how angry it makes me that I have been told thoughtout all of my genital corrective surgery history (3 to be exact & considering a 4th) by fellow trans women & doctors that anything between 3.5-5 inches is enough bc thats the average cis vagina length and the average penis size blah blah blah. Its all fucking bullshit because:

1. Yes the average cis vagina length may be 3.5-5 inches …. AT REST!!! They are fucking made to STRETCH in order to birth babies and take a dick (which come in wildly different sizes.) So anyone that thinks this shallow little non-stretchy orifice made with penis skin stuffed inside you will suffice for male penatrative sex (WHICH SHOULD NOT HURT & should be as ROUGH, HARD, DEEP as YOU want it to be should be up to you & YOU ONLY!) are mistaken. I’m convinced all these surgeons & trans women gaslighting us into accepting this are either lesbians or just don’t even have sex.
2. Nobody ever stops to actually ask you what YOUR specific wants & needs are. Maybe you happen to have a partner with larger penis? or maybe you are single and want to be ready for whatever you come across without having to worry about PAIN and bleeding during sex & both parties being extremely let down? Maybe you like rough sex? Maybe you are a size queen? Nobody should just decide for you what is best for you based on THEIR personal beliefs and standards.

Disclaimer: I'm a cis woman, my partner is trans, we're researching our options and I've attended a seminar from one of the hospitals in our area that does bottom surgery. We're making decisions about how the next few years will go and which surgeon she would like to go with, that's why I'm here. So please, take everything I'm posting with a grain of salt and take the words of support/advice of everyone else here with more weight.

I'm really really sorry you went through this (briefly peeped your post history), however, I do have a few things to say:

Firstly, I absolutely agree that different techniques should be in the conversation when it comes to bottom surgery, however, not everyone is lucky enough to have the finances/opportunity to pursue all of them. Every technique has its benefits and drawbacks. The surgeon however, does have a responsibility of reviewing your personal goals and building realistic expectations about your results.

Secondly, your claims about cis women are rather untrue. According to one study, the cis vagina is about 3 inches in its baseline state and 4.3-4.7 inches aroused (link to an article here). That is true with myself as well, my vagina at rest is shorter than my middle finger, and when aroused, can just perfectly accommodate my gf who's 4.5 inches. If we use even a marginally bigger toy I bleed, as trauma after rougher sex is incredibly common. Birth is traumatic, tearing is common, and the area can be distended and swollen for up to 6 months after. Many women have permanent changes to their anatomy after birth, not to mention complications like fistulas, bladder leakage, and prolapse, it's not the default state of a vagina to be able to go through birth. Of course, there are women who can accommodate very large penises, they are not run of the mill, and it absolutely does hurt.

Finally, I find that your accusations of lesbians/people who don't have sex gaslighting you quite unpleasant. People who are in same-sex relationships and/or aren't as sexually active can have expectations about bottom surgery as well, and we are not your enemy. We aren't lesser than you because we don't have traumatic sex with straight men with giant cocks (and what sounds like little consideration). Instead of coming at groups of people with generalizations, how about seeking out partners that are willing to be kind and work with your anatomy while you embark on the journey of meeting your surgical goals?

You make it seem like all methods are equally available, funded, and have the same level of risk. Depth and penetrative sex are things to consider and there is a difference between methods in this regard but it’s a sliding scale with overlap depending on surgeon skill and each girls body. Colon is much more invasive, it’s a longer surgery with more risks and less options for revision. It’s not well studied for long term health effects, has slightly higher complication rates and for reputable surgeons will have more restrictive pre surgery health requirements so more women will be preemptively excluded.

Anyone declaring a single methodology the best in all cases for all women is not being particularly fair to the realities people face when trying to get these surgeries. I agree it should be in the conversation, more insurers should cover it, more surgeons should train to provide it, and it should be better studied and refined.

Sorry you have a had a bad sexual experience post op, you’re not alone and it fucking sucks every time. Just because your experience has been bad doesn’t necessarily negate the experience other trans women have had with piv or ppt, but it also doesn’t mean every woman will be sexually satisfied or be able to please their partners the way they want and the “it will be good enough” that gets tossed around is not always true and can be harmful.

I’m sorry you have had such bad experiences and I hope this post helps other women really evaluate their options to avoid some suffering.

Stretch to an extent, but not very much, not at all what you're suggesting.

Cis women have a cervix at the end of the vaginal canal, a dick can only go so far. That 4 to 5 inch depth is at rest and during intercourse.

Some cis men have even stated that trans women in their experience can have deeper vaginal canals in comparison.

You didn't have a great experience and I'm sorry for that, but fear mongering and claiming that people are "gaslighting" is not the way to go about this or to deal with your trauma.

Pain is a complication, that is something you might need a revision to deal with unfortunately. Or you might need to see a gyno for nitrate treatments. Maybe you just need more healing time? I'm not sure how far along you are. That is something you should discuss with your doctor and surgeon if possible. And if you didn't have a good experience with your surgeon, try making arrangements with a different one if possible.

Accusing the community of gaslighting you and then purposely indulging in borderline transphobic and gross characterizations of surgery is not a healthy way to deal with this.

Hello everyone, this is my fisrt post and I want to share my progress with all of you.

In the 2016 I get my srs with penile inversion surgery in Guadalajara Mexico with Dr. Aguilar (cuz Im from Monterrey, Mexico) it was a disaster, no asthetical aparence and no depth. So I get depression and I feel very bad for this.

This year I meet to Dr. Carlos Guerra in Monterrey Mexico so I decided give me a last chance, yesterday was my last day in the hospital, and I feel very very happy with this process. I got peritoneal revision plus labia majora and minora reconstruction. With this dilatation I dind feel pain and was very very easy for me.
