I think "clean bill of health" is Nick's interpretation of what happened. The reality could have been very different. Nick comes in looking sick. They ask questions. They do tests. The tests don't necessary show that he is in good health. The tests often can only show that nothing can be medically done for him. Tests are often not about giving people clean bills of health. They are often a second-level search for something wrong that can be treated.

In the context of Nick's supposed reason for the visit, they were determining if the drugs he is on are damaging his liver to the point where he should not be given more of those drugs.

Did Nick share with his doctor that he is explicitly mixing provigil and his other medications with alcohol on a regular basis? Probably not. Because if he had, the doctor would have given him a lecture on the risks he is taking. Did he tell the doctor that he takes provigil at night when drunk before going to sleep? Probably not. The doctor would probably have an opinion on abusing a drug to go to sleep by reaching blackout drunk.

Since the only medication I am aware of that he currently uses is the Provigil (trade name for modafinil), it is probably that. However, curiously Nick says "one of my narcolepsy meds", maybe there's something else now?

Even if everything Nick says here is true, it is hard to avoid concluding that his doctor noticed the same things that everyone watching his streams has, which is that he looks disheveled, frail, gaunt, and so on, and feared that there was an underlying medical issue.

Nick getting a clean medical bill of health does not change that. It would only mean that there was no underlying medical cause of the dramatic decline in his appearance and that said decline is due to lifestyle changes which will catch up to him eventually.
