I don't know, heart disease can be hard to identify and while you can manage it, it's irreversible so it becomes a matter of trying to get it under control and slow the progression.

Congestive heart failure on the other hand, I think if you can catch it early enough at best you might get a couple more months together. Sometimes it's just time to go, and keeping an animal alive with a reduced quality of life is kinda selfish even though it hurts so terribly to lose them.
