From fragmented bits and pieces of my erstwhile medical training, I vaguely recall something called "moon face" being a side effect of certain types of steroid-based medications so I guess the hormone imbalance screws with the water retention in various parts of their bodies. I think that sort of issue makes feet swell up too, though I don't remember all that much about how and why this stuff works the way it does.
