Really? To me they go together. This is exciting and appealing to a man who's in his economic and reputational prime with low sexual experience. He's given time to think it over. They eventually agree to do it.

This is where he begins to over-the-top lust after Mandy. Kayla tells him to tone it down as it crossed the line into disrespectful since the audience cannot be aware of their open marriage/swinging for the sake of Nick's career and both their reputations. They test the waters even further at Hedonism II, having sex in front of others, watching others have sex and engaging in high level exhibitionism. He makes contact with the two black men with a promise to return. Between trips both Nick and Kayla have doubts about how events have transpired and the decisions they made. Cracks form in the marriage.

Nick can't help but leak the changes in his life and mentality (premium cope) to his audience. Talking about how they're valid and can be saved is sublimation of his desire to save Kayla. Nick now sees his wife as more of a whore, and begins fraternizing with Onlyfans girls more often. How he views women, men and relationships changes. He feels a need to share his views as advice. Both parties know they can't go back, so they just go forward with all the justifications they can possibly muster. Nick begins soliciting escorts and they arrange swinging encounters or attempts with other couples. He understands Ralph more than ever. He starts lashing out even more, sealing his fate as an outcast with outbursts towards Eric July and Sean.

He clings to the people he has left with all he can. Nick's substance abuse reaches critical levels and be begins to self-harm by excess drinking and not eating. He abuses Provigil to function. He hates what he's become as his money dwindles and all he has left is the bottle.

Kayla's mental health and self-image collapse carrying it all, necessitating Nick maintain a more erratic schedule as he's overwhelmed. Economically they suffer further. They are unable to financially and professionally maintain some of the help they had. Nick can't help but wonder why nobody is helping him - all they do from his perspective is lie, attack, criticize and embarrass him. In a fit of anger he lashes out at John, a perceived betrayer.

His mental health is destroyed, he's hurting his own body, and his marriage is close to shattering. Simultaneously his public persona is a joke, his personal community is no longer warm to him and he knows he's made mistakes you can't come back from. He's not capable of making decisions that will serve him and his family. He embarks on the path to a prolonged suicide. [/ISPOILER]

Originally the creators made it to be "Adderall lite" but because of laws, it meant that you needed a shit load of prescriptions and having to jump through doctors to even get it. A little biotech startup optioned it from a French company who owned the patents - it became a billion dollar a year drug. Fighter pilots use it, Himalayan mountain climbers use it (I think even Ed Viesturs mentioned it in one of his books), of course 3rd shift syndrome and narcolepsy. It ended up becoming a smart drug for college kids despite the fact that the FDA denied it for ADHD.

Here's some deep lore - the former CEO and founder of Cephalon was a Temple Alumn so of course the Temple University newspaper made a flash parody video of him (he's since passed away - heart attack).

Thoughts on Nick -
There are starving African children less emaciated and more muscular than Nick is. I think that's a big part of his issues - he's never lifted weights, doesn't do any landscaping work, doesn't do any mechanic work. Basically nothing involving manual labor, which for a man is not a good thing.

If I also had to guess on illicit drugs it'd be GHB. That's also big in the gay scene or if you're a fan of Delray Misfits like I am is how you learned about the insanity of that drug.

The Theory

When discussing his medication review Nick says "one of my narcolepsy meds" and "I don't use it very often but you know I have to use it a little more because of circumstances changing".
If we believe these statements that would indicate either:
a) The review was for modafinil and Nick is not taking it daily. He spends his whole day very sleepy because he has narcolepsy. (And is drunk.) Which would beg the question why isn't he taking the medication as advised to help stay awake?
b) The review was for modafinil and Nick is not taking it daily. He spends his whole day with an acceptable level of wakefulness (for being drunk) because he doesn't have narcolpsey. Which would be suspicious to anyone aware he isn't taking his medication daily.
c) The review was not for modafinil because he takes it daily and the review is for a different medication.

If the answer is outcome C then we should probably be concerned with what the other medication is.
SSRI anti-depressents are often prescribed to narcolepsy patients because they inhibit REM sleep (narcolepsy is essentially dysfunctional REM sleep) however they would also be something you take daily.
I suspect it is most likely a more potent stimulant such as Ritalin/Concerta (methylphenidate HCl) or Adderall (amphetamine/dextroamphetamine) prescribed to be an occasional booster for when he is having a particularly busy day. I think it's probable Nick's stimulant usage has increased and he asked his doctor if he can have a larger prescription of his stronger medication under the pretence of whatever crisis he is currently dealing with. The doctor, seeing that Nick is in declining health and smells like whiskey has insisted on a full set of tests before signing off.
The last option is Xyrem (sodium oxybate, a GHB salt). Given that Xyrem knocks you out and seeing as Nick would have you believe his current life mission is to stay awake for eternity so he can chauffer his kids around the state. I would have thought its only interest to Nick is GHB bragging rights.
