Usually the disabled that can't drive (blind, physically disabled and unable to get any car retrofits, etc.) get to have special vans pick up and deliver to them to the relevant places immediately or almost immediately. You know...like cars.
They make even make special vans for the physically disabled to drive. My aunt is paralyzed from the waist down and she had a van she can roll into, slide into the driver seat and operate the thing with only her arms. Granted, riding with her always made me very nervous.
The only example I can think of where public transport is a good option is the Taipei MRT. If you are blind or wheelchair bound and don't have an aid of your own they have people who would make sure that you get on the right train safely and radio ahead to your destination so someone is ready to help you off and get you to the right exit. Now walking around after leaving the MRT is a nightmare because Taipei sidewalks are haphazardly put together so they have random steps/drops, stalls, scooters, benches, and other crap that I can't even fathom a blind person walking around safely.

It already doesn't work for most countries with a subway system (yes, even Eurozone ones). Several train systems in places like France, New York and London don't have a elevator at every station meaning you have to make lengthy detours around very old stations and perhaps have to wheel your ass through dangerous places like refugee ghettos.
As a bong I can confirm that most stations aren't built for the disabled in any way. At my local station you need to climb a set of stairs to get to the other platforms, it's not at all better for disabled people since they aren't even able to access 2/3 platforms and the situation is pretty much identical at many of the nearby stations apart from the very rural stops where you have to go over a crossing on the track itself to change platforms. I know that Americans love to bitch and moan about their country but your government clearly puts a lot more effort into accessability than anyone in Europe does.
