Don’t you have to fast prior to surgery? To prevent aspiration? I reckon her not being able to have snacks might be as big a threat to not getting it done as the anaesthesiologist.
You have to fast, usually nothing to eat after midnight, clear liquids until a few hours before surgery. Different surgery centers and hospitals will have slightly different timing, but it'll be something around that for an elective surgery. They will almost certainly have written instructions, whether a webpage, a patient portal message, or photocopied preop instructions. Probably a combination of all of those.

However, it's likely that J will be staying at the AirB&B the night before surgery, then checking into the hospital early in the morning. When they check you in, they ask the last time you had food, and the last time you had liquids, but they don't shove a dipstick down your throat to make sure you aren't lying. So you're right, that could be a fun wrinkle!

Black people have slightly higher bone density, so of course having a BMI of 100 is fine. There’s probably something to be said for taking ethnic origin into account. A tiny Vietnamese lady will be seen as overweight at a different point to a 5’10” Dutch white women. There are ethnic differences, but they’re not that big - it’s a few points around the boundaries. (Also significant differences in when you get diabetes, Indian people seem to have drawn the short straw and get diabetes at lower weights.)
I love the screeching about BMI. Now yes, BMI isn’t exact at fringe cases, a very short woman who is very fit and slim but has some muscle can be seen as very slightly overweight when she’s actually fine, or a very muscular athlete can be seen as almost obese when he’s just jacked. BUT these are cases where it’s a couple of points over the line. Maybe that girl has a BMI of 25.5 and she’s just short and can throw a bale of hay around. They always use the Rock as a case of a guy who has a BMi just in the 30s as a ‘proof’ of why bmi should be abandoned.
NOBODY with a BMI of 40 is healthy. Man is built literally like a rock. He’s not fat.

Jay will very likely have to have a pre-op visit with the anaesthetist. No medical professional is going to put her under without prior and current examination.

Definitely; that's why I mentioned the anesthesiologist running screaming when he sees her neck.

But when she rocks up day of procedure at 0500 and some nurse or aide asks her the last time she ate, they're just going to write down the time she tells them. J's paradigm is that words only mean what you want them to mean and feelings dictate reality. Clipboard lady thinks that time progresses in a linear fashion and is going down a pre-op checklist for safety.
 
Surgery or not, this will be used to pontificate about said staff being an oppressive shitlord. The only possible exception will be if staff is a troon. J may let them get away with it then.
The practice she’s going to does have a Troon surgeon (not the surgeon she’s seeing though), so there’s a pretty high chance they have other transgenders on staff.

I think they're claiming that being called "obese" in the negative has been traumatic in the past, and so hearing the term now is triggering. Which isn't how trauma works but these people are cuckoo.
I don't get how it's totally fine to sell shirts reading FAT BITCH, but hearing "obese" will cause you to have a trauma response like you're a child sex abuse victim or Vietnam vet lol. I guess "obese" isn't edgy and cute, and just hits a little too close to home in the medical sense, ie, your diabetic neuropathy riddled extremities.

Similarly, "obese" coming from a term meaning to eat/devour is like....yeah, and? Everybody knows how you got this way.

Love the straight-up "racially motivated" accusation re: BMI. It isn't enough to point out the outliers noted by @Otterly - no, it had to have been a bunch of evil white doctors sitting around being like "lmao fat ass niggers!!"
(Have they considered that if whitey was truly evil, he'd want non-whites to be held to a lower standard in order to pass the military BMI requirements and be sent off for cannon fodder? These people are ridiculous)

It sounds like that surgeon should look for better office staff. Either they're terrible at estimation, or they're bad at preauths and coding, or both.

The surgeon doesn't actually want troons dissing him on social media, and you can objectively prove bills received a lot better than "man this vagina he installed sucks" so it's harder to call that slander.

The No Surprises Act covers self-pay but there's something going wrong in the paperwork if multiple insured patients are told they'll be covered, then get a much bigger bill.
