I think its because these are obviously young women/girls with immense self image problems, cutters and generally self loathing psychologically disturbed kids, and "Dr" Scythe is a legit fucking psychopath.
Its quite obviously just another form of self harm for a lot of these girls, and the very obvious cutter scars would disqualify them from being competent to consent to this sort of shit if there was even the
tiniest amount of oversight or ethical standard here, but these Fleshcrafting Homunculi fuckheads like Sidhbhbhdh are in this purely for a paycheck, thats why she'll take even goliaths like Blowhole Rylan, and fucked up cutter girls like the one in that picture.
She carves these extremely ill self hating basket cases up, and if she does a half assed job and something goes wrong she's deliberately opted out of malpractice insurance so they can't even sue (something utterly bizzare to me, that a surgeon can legally operate without insurance).
In one way its their own fault, stupid delusional Pooners, but looked at from another way these are just lost, miserable little girls that have been betrayed by every responsible adult in their lives and the medical system itself.
