It'll never not be funny to me how Zack used to be active in the atheist youtube circle, and would mock anyone who claims to feel god's presence and faith, but wants people to believe in his inner gender soul existing. Also funny how he wanted religious people to prove god's existence, and yet if someone asked him for proof that hes a woman deep down, he wouldn't give an answer except for his say so.

I know two ancient dudes who got the one knee at a time double knee replacement thing, both times outpatient, and I suggested this was kind of sick and cheap, and they said they'd rather be anywhere than a hospital.

(They generally don't do both, like they don't do both eyes with Lasik.)

Yeah, same. Although ortho is a lot less worrisome to go home with than a potential gusher like a hysterectomy or thyroid. They're not waking you up in the taxi; you still have to graduate PACU and then the physical therapist checks you out again in Short Stay, which is more time to decide that you need to be inpatient after all. A crusty old person who's been dealing with chronic joint pain is more likely to be able to cope after work on that joint, and to be highly motivated to go back home to cable TV and their dogs.

The trick is pre-op assessment of which patients are self-motivated and intelligent and have enough home support to be safe day surgeries, and which people will need a social admission--that's on top of who's medically complex enough to need observation. Surgical centers are really serious about this, because they can't go home until you do; operating in a hospital makes it more possible to convert outpatient into an admission.

There have been some big advancements in local anesthetics recently, new long-acting agents for local or nerve blocks (to the tune of multiple days) and the "pain ball" elastomeric pumps that are non-electronic and continuously infuse local anesthesia through a subcutaneous catheter, even in the privacy of your own home. Sometimes we can have nice things!

The two I knew also said they'd have done both at once but they were advised otherwise. They said it hurt like hell but the knee worked so well afterward they wanted the other one working too. Of course these weren't giant pussies, either. One of them is now 80 years old and spent his last summer building a wall, just because he wanted one, and is now going to spend next summer building another wall, just because he wants another.
