Yeah I reckon it's more to do with his overall poor health. Being an alcoholic, his body cannot absorb nutrients - and of course he didn't even try to eat properly. He stayed inside and slept very little - that's super bad for immunological system. So then the skin gets dry and easily irritated, your immunity is fucked so if you scratch it, small infections occur, you're high on coke so probably picking on skin, scratching off scabs.
Not to mention the state of his greasy nasty hair, that was a great indicator of how lacking in hygiene he was personally.
And the state of the house, dust, dirty clothes - that's just breeding ground for microbes.
