He was on life support. It's not at all unusual for a patient on a ventilator to get pneumonia, which complicates the treatment of whatever was going on that made him need a ventilator in the first place. Ventilator-associated pneumonia is a very, very common cause of death in ICU patients.

I get what you're saying, but I don't think encouraging snorting or smoking it as opposed to injecting is considered "harm reduction" because of relative overdose risk. I was always under the impression that the harm being reduced was just the risk of air or talc embolisms, abscesses, pericarditis, transmission of hepatitis/HIV, and all the rest of the problems that come with injecting who-the-fuck-knows-what with filthy rigs, questionable injection technique, and no fucks given.
