health care professionals have got to have a way to communicate the reality without patients getting pissed off

I've been told it's done by omission. :story: When I went through cancer treatment a few years ago, the oncologist would send updates to my primary care physician that always stated up front that I was "pleasant and cooperative." I'd crack up every time I got a cc:. During a slow evening in the PACU, I got to ask one of the nurses why that phrase was such a thing.

He said the phrase didn't mean anything when it was included. But when it wasn't used, it was a warning to the other doctor.

I am haunted by the possibility my life will yet hang on a missing mystery word. Should medical missives always include "acquiescent" but mine doesn't and, burden on polite society that I am, I will not be resuscitated? I has the Anxiety.

In my country, you can demand to see your entire medical record, and your providers have to supply it... unless it's psychiatric records, and in that case, they can refuse if they think what's written there will be so distressing to you that it would "prompt another psychiatric episode".

Makes you wonder what gets written in those.
 
(My mention on her parents/insurance is me just wondering if potentially you can pay or contribute a stake towards another party's insurance, such as an adult sped child, in the US, which I know nothing about.)
From some cursory research: in New York state, coverage can be extended to age 30 for unmarried adult dependents who aren't covered by Medicare and aren't eligible for other health insurances. However, Stephanie's older than that, so regardless she's not getting on her parents' insurance. I'd bet dollars over donuts she's on Medicare/Medicaid.
Medicare, for all the non-USians here, is government subsidized healthcare for people older than 65 OR disabled people who can't work. There's also Medicaid, which is similar, and covers people under 65 who have limited income. Both Medicare and Medicaid are paid for by tax dollars; Medicare is federally funded, Medicaid is funded jointly by the fed and the states.
Both of them fucking suck. They're confusing as hell to navigate, constantly threatened by politicians who don't know what being poor is like, and most providers don't accept Medicaid/Medicare because it doesn't pay out as much as private insurance does. It's hard enough finding a regular provider or primary care physician that takes Medicaid, but the more specialized of care you need the longer you have to wait, the further you have to go, the less the government will cover, and the more hoops you have to jump through. Stephanie's "poors with bad teeth" comment stems from this; an overwhelming majority of dentists don't take Medicaid at all. Scarier still is when Medicaid refuses to cover things like insulin, which are lifelong and you cannot skip doses of. I can't get into it or I'll be raging about Medicaid for days, but the short of it is that it's terrible and you have to have it or the government fines you annually. Thanks, Obama.

Being "pleasant" isn't standardized by any means; sometimes it's just a shibboleth of when/where a given doctor trained. Or if you've worked with Dr. Smith for years and all his referrals are "pleasant," then you know to look out when one isn't.

I have thought about waving a printed consultation and saying "look, I've been officially described as 'pleasant,' in legal documentation! That means you have to enjoy my company." That's probably not what pleasant people do, though.

From anecdotal observation, you have to be over a certain age to be "delightful."

There isn't a whole secret language of care based on one or two load-bearing adjectives, though. If someone's grossly noncompliant or skipping appointments or threatening staff, that's going to be spelled out later, and in the "plan" section, since it is a barrier to care. The provider isn't going to repeat what's technically hearsay about the patient cussing out their office staff, but they may reference a behavior contract. I've seen many providers note in their own dictation that a patient was let go from a specialist or a previous PCP's office for no-shows.
 
late & gay re: asexuality: there's a subset of terminally online people out there who won't shut up about being horny and have porn blogs full of Hazbin shit and other depraved cartoon fuckery. I genuinely believe these ones when they call themselves asexual... because what they mean is that they don't experience sexual attraction towards actual real people. Essentially, we've raised a generation of kids in such a socially stunted way and let them pump their eyeballs full of so much high-octane extreme fantasy porn while they were developing sexually that they now look at real life human beings and legitimately feel nothing. We don't have a word for "freak who is only attracted to cartoons" so they pick "asexual" instead. Pathetic and sad but what isn't nowadays.
This pretty much sums up Maia Kobabe, author of "Gender Queer" which is autobiographical - she calls herself asexual and non binary because she rotted her brain with so much gay fanfiction as a teenager that it left her unable to have meaningful relationships with real people and gave her a sense of alienation from her own body - she's clearly a straight woman who just really enjoys reading about two men fucking, but spins it as "I mustn't really be female if I relate more to fictional men". Half the book is her talking about sex, her fantasies, and the ways she likes to get off. Being raised by hippie parents who let her do whatever the hell she wanted and neglected to teach her much about the birds and the bees certainly didn't help matters, either. The thread where it's discussed is certainly worth a look, especially because she's rather similar to Staph now I think of it.

I'm still convinced she's only saying she's really into metal and folk music because it fits the image she's trying to present, when we all know she's probably still into MJ and Coldplay. She hasn't mentioned any metal bands other than Rammstein or Ghost either which isn't helping, and when it comes to metal those are like entry level stuff. Back in the day she would have been called a poser.

Folk music, she could play and sing herself; it's folk music. You can even sing a capella in a group; remember the sea shanty/folk fad mid-pandemic? Folk music, folk herbalism, folk magical practices; it's not a reach to see how that could all come together in a more honest public-spirited pagan than our Stephanie.

If she were truly into metal, one of her stated "transition goals" would have been that she wants to do a better Cookie Monster voice.

Interesting, the doctor who called a patient "unpleasant" was an oncologist and I'd see the "so-and-so is a pleasant, 45-year-old woman" a lot from other cancer docs as well.

It was UN-something and she really, really was. Miserable ol' bitch. But hey, she was an elderly cancer survivor at the time so she's likely long dead by now.

I had a "patient tolerated procedure well" in my notes once. I did wonder what that meant and what the threshold was for tolerating a procedure well vs tolerating it poorly. I don't think Stephanie is capable of considering what the nuances of words mean-- in her medical record or anywhere else.

As a sidenote: she mentioned My Friend Dahmer again. I read the graphic novel back when it was new-ish, and I don't remember feeling any sympathy for Dahmer at all based on that work. I have not seen the film, so maybe it's different (and I realize it's "different" insofar as it involves an actor that Staph wants to bang). It was an interesting perspective-- a horrific person emerges from a mundane suburban setting-- but I wouldn't have called Dahmer sympathetic. I'm half-tempted to reread it and see if I can see what Staph is seeing.

Generally, no screaming, flailing or fainting. But it's less a commentary on your stoicism than it is for Future Lawyers, the ghost of whom hangs over most modern charting.

Nothing out of the ordinary occurred during the procedure, so even if they won't be able to remember your specific case in three years they can point to that note and say they clearly weren't given cause to think that later you would be mentally traumatized or have any weird sequelae warning signs of which should have been caught at the time.

It was popular among high school kids larping DID to make these edits of what the fictional character they’re larping really looks like versus how they see themselves, like changing the color scheme or hairstyle. Is this person they’re interacting with a teenager?

I don’t know much about kinning and I think they sometimes use the term source, but in recent years the fake DID craze co-opted it for when they pretend to have a multiple personality that’s a direct copy of a fictional character or real life person, and they point to the media they “formed” from as their source, other alters from that media their “source mates” and pretend to have pseudomemories of things like for example pouring acid into a kid’s brain, and calling them “source memories.” I’m sure My Friend Dahmer blurs the line for these people between fictional source and “hey I’m pretending to be a real person who tortured real people.”

It gets to the point where DID larpers don’t want to interact with other “alters” from the same source, that makes them uncomfortable or triggers their trauma in some inarticulable way. I guess there’s only so much attention to go around and it’s not fun anymore if everyone pretends to be Jeffrey dahmer.

They're such similar online subcultures it's probably not even worth splitting hairs about which is which - both come from the same place of wanting to be special and unique, unlike the rest of those boring white girls (because let's face it, that's who makes up the vast majority of these people) as well as an unhealthy attachment to a particular character or indeed a real person. Only difference is the DID fakers are trying to legitimise it through attaching their identity to a rare mental disorder, "otherkin" see it as more of a spiritual thing, or compare it to having a gender identity.

It's like when they took three guys who each thought they were Jesus and put them together to see what would happen. They wrote a book about it. The guy running the whole thing thought it would make the three men snap out of it, but in the end it just made all of them crazier.

Of course it's a teenager. It was a teenage "zoo" who she was speaking to before she threw a strop on the other account. The only people generally idiotic enough to be into this shit whilst also not active enough to be actually in prisons for it - as in non lazy adults, who don't just live entirely in fantasy - are always going to be teenagers.

She's right back in her comfort zone now, now more "humble" "hiding" - which means the next part of the staph cycle-the best part, the blow up- is right on schedule for soon.
