I don't think it was ever confirmed it was the sister because Sam Hyde never explicitly said it was, but he said that a family member hit him up to say that Anisa doesn't care about alzheimer's and that she has a strained relationship with her dad. Most people assumed it was probably the sister who is known to hate her.
