In any case, I can't claim to speak for everyone I know, but I don't think people tend to include themselves when they use that expression.

I can't even say for sure that Trump's looked healthier because he's yet another one case of fat, amorphous blob. You can't tell what's going on under the baby fat he carries on his face. Besides that, the combover, tons of makeup, etc.

There was one candid shot of Trump from a while back in which he looked very much like a senior, just good luck finding it when if you type "Trump" in Google Images all you get is shooting and news pics.
