Can someone tell me whats on his head? It looks like a wrap to keep the jaw closed? I see what I assume is a cpap or other type of breathing help, but is the thing under the jaw and around the head just to help make that work better?

I think it's probably BiPAP, and the strap around the chin is to gently hold his mouth closed so that the "positive pressure" is maintained. If air is able to leak out the mouth, the lungs can develop areas of collapse, called atelectasis. It's really common in premature babies (and other patients with sick lungs).

Edit: I was going to say that it's good he's healthy enough to avoid intubation so far, but quite honestly, sometimes death is a mercy.

I have a physical disability and his toes look a lot like how mine did when I was a baby. Large gap between my big toe and other toes, with the smaller toes kind of just moving as one toe. I'm thinking he has some form of brain damage, which is why they are kind of contorted like this. That would fit well with the general floppiness too. I wouldn't be surprised if he ends up with foot drop. Probably the least of his problems.
