It's the ultimate reduction of what modern American-style big business mindsets are like.
Something I've noticed is that (big, huge) companies (some private, most public) are so obsessed with continuous never ending exponential financial growth they can't get it through their dense greedy skulls that there's effectively a market cap of
A) People and
B) Shit people are willing to put up with before they get mad enough to do something about it.

With other companies, say food companies, the outcomes might not be so extreme, there's a lot more willingness to just look for an alternative that while still screwing the customer, screws the customer the least.
However seemingly what insurance big wigs seem to forget is that they're dealing with people's lives, health and in some cases houses.
Believe it or not, people have some semblance of self preservation, and if you show that you're actively going to work against that, then you shouldn't be surprised that some of said people see you as an active threat to their self preservation.
And being as powerful and influential as execs are, said people might feel it's a nigh-unwinnable battle, which in turn makes them feel like they have nothing to lose anyways.
And that's where you get stuff like this happening.

The way he operated the gun and not taking an extra shot and the extremely high level opsec, exit plan and only slipping up once because a chick was flirting with him (he seems to be handsome and fairly youngish) don't really scream "this evil health insurance company was the reason my family member died".

This guy apparently single-handedly was able to do all of the above, use burner phones, not fuck up his cover for 10 entire days (that is a fucking long time), know exactly which mode of transport to use (greyhound buses), pay in cash, presumably steal an ebike (ebike still hasn't been recovered), pack several different but semi-similar looking jackets and also put monopoly money in a backpack for shits and giggles? And not fuck any of this up for 10 days? And to also ditch a burner phone at the scene of the crime? And also skedaddle in under an hour in a clear, methodical and planned out way to exfiltrate from New York City via a greyhound bus that has 7 or 8 different stops?

Everything about this to me screams paid operator. Because people that are emotionally involved make mistakes and slip ups and get impatient or greedy or stop over the body to take that extra pot shot at the body or hesitate for even a second or have the adrenaline rush impact you at all.
And if you're going to get a paid operator then you can find people from overseas.

There's only 3 other options in my mind:
