My guess is that this latest therapy talk is because Salah is getting more nervous about what he committed. She sold him on a youtube travel vlogger lifestyle and that she would change her behavior. Now this arrangement is getting more strained as there's no sign that anything will change despite her promises and reassurances. The whole phone controversy was interesting because it seems like one of the few times he put his foot down. She also started driving which could be because he's maybe being more skeptical about what she orders and with a car, he can't really stop her from buying something extra along with her "healthy" meal.

So therapy is probably a way to sell him on her changing for real this time. It won't happen but he's still clueless. From her perspective it can give her ammo for excuses the same way she does with the whole eating disorder clinic telling her to not restrict in any way.

If (and it's a massive if) she has been using all her youtube income to pay off her tax debt like she says, as soon as it's done she could just be herself and tell him that if he doesn't like it he'll have to break up with her.
