What really rubbed me the wrong way itt is, that people are excusing the staff, I think we're forgetting, that this sicko still let's the good doctor work his magic, instead of neutering his "husband" himself.

The only similar thing that comes to mind are the court eunuchs in East Asian societies, though I'm pretty sure they did it as a way to elevate the standing of their family/community by serving someone important (or the emperor himself) and had nothing to do with deranged fetishism.

I'm not a historian but I know a little, in pretty much every culture it wasn't really an elective thing, it was forced. Mostly by virtue of being a slave or child, though it was sometimes a punishment as well. I remember reading in a class on Chinese history some official was punished for some crime and given the choice of death or becoming a eunuch.
