I think the important difference is that these depictions of gross acts on children and siblings are used as horror material, and to communicate to the reader that the characters doing this shit are bad and crazy people. This is not the case for KCD. In fact it's the opposite: they are soulmates and this is a GOOD and normal thing you are seeing!

Dragon Age (Origins, awakening) had decent romance systems. My first playthroughs i did not even notice the systems because they are not forced at you, and the games had so much other stuff to offer. Some years later I did romance Anders. But the difference is, the process was much better designed than we get nowadays. I remember actually feeling giddy when I gave him a gift he liked and he said something cute or shared more lore about his backstory. Romances in vidja can be done well, and when they are done well, I don't mind them being an option.
