Benzoylecgonine can be found outside of the human body, such as in bodies of water and so on. How it metabolised or rather how it was created is largely unknown to us. It's not something we care about. It's possible to synthesise benzoylecgonine in a laboratory setting, many products and patents exist. I would assume high purity samples are used to produce calibration curves for the GC-MS instrument.

Any cocaine that got into her hair would have been washed off during the routine procedure. Whether it's environmental trace deposits due to skelly's filthy coke den or the cop rubbing coke into her hair while wearing gloves.
