The "Sophie" character was created as a means for him to escape his former identity; an easy way to attribute all of his past failings as a byproduct of being in the 'wrong' gender. This was discussed at length with Haselgrove, and the reason why she (rightly so) denied him hormone treatment.

Autphag may come across as reasonably intelligent (or at least verbose) when he posts, but when dealing with him face to face it's very obvious to see what motivates his desires. He lacks the ability to read or perform social cues appropriately, so it's not difficult to gauge what's rolling around in his head.

The core issue regarding the lack of progress in his sessions with Haselgrove was his combative, deceptive, paranoid nature and unwillingness/inability to share any kind of honest dialogue with her. He seems incapable of answering a question directly or lacks the ability to even understand what that entails; any inquiry was met with off topic rambling devoid of self reflection.
