So, an incredibly common trait of narcissists is that friendships are achievements, not relationships. Kelly has two children who are desperate for her attention and a husband who is grimly hanging on for their children's sake. There are three relationships right available right there. But because they want her, she doesn't want them. They're "easy" not really which makes them worthless to her. The people who hate her though, they're a challenge, which makes them worthwhile, and interesting, and exciting. Winning their love and affection is a victory and a boost to her pride and self worth. These people are the medals at the end of a long race, and once Kelly has them, she will instantly lose all interest and regard for them. They are a mountain to conquer once and never visit again.
you're right, people do find you disgusting, but it's not (just) because you're fat. being a vile narc with murderous thoughts and neglected children are much worse crimes than being a big smelly heifer.
