Claudio Hernández Castañeda, with file number
188029 at this Institute:
The Sleep and Home Ventilation Medicine Service has been treating Mr. Hernández Castañeda since 2011. During the course of his care, it was concluded that the patient has a
Neuromuscular Disease due to Nemaline Myopathy. This condition has affected his motor function and muscle strength. However, he has experienced significant respiratory complications since 2011, at which point he began
home-based nocturnal ventilatory support.
As the disease has progressed, the patient’s respiratory condition has deteriorated, with exacerbation episodes requiring prolonged hospital stays in 2012 and 2014. This evolution necessitated a shift to
tracheostomy-assisted ventilation, in which the patient maintains an open airway in the neck via a cannula that provides access to the trachea, through which he receives support from a
portable mechanical ventilator. Although this condition can be managed on an outpatient basis, it presents a disadvantage due to the
constant risk of infection, as the tracheostomy provides a direct path between the patient’s trachea and the external environment.
In recent years, the patient’s condition has remained stable. He receives
rehabilitation care at home and continues to require
Home Mechanical Ventilation through Tracheostomy, with increasing pressure levels. The need for this support has progressively grown to
12 to 14 hours per day in order to maintain stability and preserve as much autonomy as his condition allows.
It is important to note that throughout the medical care provided to
Claudio Hernández Castañeda, he has shown an
optimistic attitude, continuously striving to overcome his medical challenges and, in spite of them, grow and improve in every way. He has completed secondary education and has actively sought ways to pursue higher education through all available means.
Therefore, I consider it important that the patient receive
support in his academic activities in a way that does
not compromise his safety, given his
vulnerability to respiratory infections due to his chronic tracheostomy condition.
