I've heard from a former prostitute that undergoing a physical examination is common before any sex takes place. It makes sense, people lie all the time, a lab test report could be faked, so if the penis looks funky it's probably got something transmittable, possibly despite the condom.

Obviously I can't say that's standard everywhere, or even if the person who tweeted about that is telling the truth. But logically I can see the point, it also would explain why the subject came up. I think Russhole made that all up to save face. I think he either tried to pull another stunt where the whore told him she would only accept $$$$, but Russhole only brought $$$ and she told him to pound sand, or it was some other straightforward rejection that Russ felt embarrassed about, so he made up the whole circumcision story to play victim and take the sting off his tender pride. It feels like the kind of lie Russhole would make, because it doesn't make any sense if you even think about it for half a second. And Russhole never thinks out his lies.
