The only even halfway normal (as in, not exclusively used by trannies) way a person might refer to an adult woman as a "good girl" would be specifically to be purposely insulting/condescending for exactly this reason.

As in, the only time I've ever heard an adult woman say this to another adult woman was about an hour deep into the free bar at a wedding reception, and it was immediately preceded by "fuck off" along with an exhaustive list of said woman's personal and physical shortcomings.
