I know a guy that works in the prison system in the US and he's told us that inmates will literally shove spoons into their urethra and cups up their ass to cause injuries and prevent themselves from being transferred out of prison to mental facilities. If, after the competency hearing, the court finds by a preponderance of the evidence that the defendant is presently incompetent to stand trial, the court must commit the defendant to the custody of the Attorney General.
