IMO the imaginary friends is just an attempt to trick people into thinking he has supporters on his side. With the fake doctors visit I think that he was trying to make people feel sorry for him so that they would buy him a system. "Oh no! The poor guy can't do vocal work anymore so he needs a new system to be able to zip zip zoom and make content". It's all just manipulative bullshit to fool people into helping him.
I also wouldn't be surprised if he's leaning into being a tranny in the hopes that he can get some LGBT supporters on his side. Not that it would surprise me if he was fully into that sort of thing given his sexual deviancy. However everything that Chance does is done for a selfish reason.
