Two of the biggest grifters I ever encountered were brothers who if they had Wikipedia articles, you can easily guess what their Early Life section would say. One was a disability lawyer and the other was a chiropractor who would always back up client claims of back pain or fibromuhalgae or whatever in SSA administrative hearings.

Dr Steve would need to be licensed to practice in Nevada if Melton had a telemedicine consult with him. Its based on the state where the patient physically is.

Now, he could have seen Melton earlier, but its still strange to get a last minute note from Dr. Steve and not from his regular PCP. Do we even know of there is an actual diagnosis or is this merely, "hey Im a doctor and this guy said his back hurt. Let him off the hook, pls."

Regarding his "back pain," he flew to Vegas, so he could have have done a stopover in Minnesota before flying to Florida.
