The 25th amendment wouldn't work like that, if a President goes "I am fine to do my duties in office" then the powers go back to him, and if they try and 25th him again and he says he's fine again, then both the Senate and the House would need to 2/3rds agree he's not fine to do his duties (which they wouldn't in numbers sufficient to meet that threshold). It's not really designed for this scenario, it's more for something like the President's in a coma.
I can also guarantee Trump's not been told "no" about firing a nuke. He's probably proposed it a few times and been quietly guided away from it. If he'd ordered the nukes to be fired and someone told him no, we would have heard about it at length via Truth Social.

I don't have insider information, and the OSINT sources I check haven't provided proof of this claim (or even a mention), so I don't know its validity. What is public is Trump's threat to destroy the entire Iranian civilization (multiple times), which can realistically only be done with nuclear weapons. While not 1:1, Trump was already nearing the nuclear threat line. It wouldn't be shocking if he started bringing up the use of nukes. It would weaken his negotiating position, however, if the only way he can pressure a country is to threaten nuclear weapons against them. That's on the same level as Russia vs. Ukraine, and North Korea vs. The World. If Iran were to call his bluff after a public threat of nukes, and he doesn't follow through, the US looks the biggest pussies the world has ever seen. MAGA would love it, though. Only the strongest men make threats they don't keep.
